29 CFR 1910.213(h)(1):     The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the material being cut:    Perfect Packaging Company, located at 26974 Eckel Rd., Perrysburg, OH: On or about October 06, 2011, the employer failed to ensure that the blade was fully enclosed while employees operated the Rockwell radial saw (Model 40-CL, Serial # DY4668), located in the warehouse, to cut wood. Employees were exposed to amputation hazards.    Perfect Packaging Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.213(h)(1)), which was contained in OSHA Inspection # 311603781, Citation number 1,  Item number 7, and was affirmed as a final order on 08/12/2008, with respect to a workplace located at 26974 Eckel Rd., Perrysburg, OH. 29 CFR 1910.178(a)(6):     The employer did not ensure that all nameplates or markings were not maintained in a legible condition:    Perfect Packaging Company, located at 26974 Eckel Rd., Perrysburg, OH: On or about September 30, 2011, the employer did not ensure that the data plate on the Clark forklift (Model CHY 300, Serial # HY2030-13-2261, Maximum capacity of approximately 30,000 lbs.) was maintained in legible condition, preventing operators from knowing the rated maximum safe capacity of the powered industrial vehicle.    Perfect Packaging Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.178(a)(6)), which was contained in OSHA Inspection # 311603781, Citation number 2,  Item number 1, and was affirmed as a final order on 08/12/2008, with respect to a workplace located at 26974 Eckel Rd., Perrysburg, OH.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Perfect Packaging Company, located at 26974 Eckel Rd., Perrysburg, OH: On or about October 06, 2011, the employer did not ensure that employees, operating forklifts such as, but not limited to, Toyota (Model 7FGU35, Serial # 70659, Maximum capacity of 7700 lbs.) and Toyota (Model 7FGCU30, Serial # 66384, Maximum capacity of 5120 lbs.), are competent to complete the task, exposing employees to struck-by, caught in between, and tip over hazards.
