29 CFR  1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by point of operation and rotating parts:   Employees engaged in cutting activities at the Grizzly 10" 5 HP Left Tilt Table Saw area of the Cutter Station were exposed to amputation hazards in that machine guarding was not utilized. The employer permitted employees to circumvent machine guarding at the table saw by removing the barrier guard on the running machine to cut the thick cardboard and to remove the last board piece. While doing this, an employee sustained serious injuries when their gloves became entangled and got caught in the blade. An employee was injured by coming into contact with a moving table saw blade while removing the table saw guard.

29 CFR  1910.1200(e)(1): The employer did not develop, implement and maintain at the workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g) and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met:   Employees engaged in making crate boxes activities were exposed to health hazards in that the employer had not developed and implemented a comprehensive hazard communication program. Hazardous chemicals used at the workplace include but are not limited to S-19036 SILICONE LUBE ULINE (lubricant), and glues.
