29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiant agents: a) Juan Orgota, Findlay, Ohio: On or about October 13, 2011, at address 7217 Ashbury Lane, Findlay, Ohio, the employer failed to ensure that each employee was provided appropriate eye protection while engaged in nailing activities, such as securing shingles to the structure roof with a pneumatic nailer, exposing employees to eye injuries from flying particles.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above the lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems nor was another provision in paragraph (b) of this section provided: a) Juan Orgota, Findlay, Ohio: On or about October 13, 2011, at address 7217 Ashbury Lane, Findlay, Ohio, the employer did not ensure that employees were protected from falls while applying new roofing to a steep sloped roof on a residential type structure. Employees were exposed to a potential fall of approximately 10.1 feet to the cement ground below.

Employees were exposed to falls of approximately 10.1 feet to the ground below, which could result in permanent disability or death.
