29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:    On or about 10/21/11 during the shingling of a residential structure, the employer did not ensure employees used fall protection systems/devices and exposed employees to falls that ranged between 10.5 to 15.5 feet.      Note:  The employer was not following the rescinded residential fall directive STD 03-00-001 or the general residential fall standard concerning fall protection for residential construction.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about 10/21/11 during the shingling of a residential structure, the employer did not ensure employees were able to recognize or abate fall hazards associated with residential construction.  Employees were exposed to falls between 10.5 to 15.5 feet.
