29 CFR 1926.417(b): Equipment and circuits. Equipment or circuits that were deenergized were not rendered inoperative and did not have tags attached at all points where such equipment or circuits can be energized: (a) At the project on or about October 27, 2011, and at times prior thereto, there were no tags on the circuit breakers that were to remain in the off position in two circuit breaker panels. The breakers supplied 110 VAC power to ceiling lights that had been demolished. Wires protruded from electrical boxes and conduit that were about 7.5 feet from the concrete floor. Employees handled 12 foot long steel studs near the protruding wires.

29 CFR 1926.405(b)(2): All pull boxes, junction boxes, and fittings were not provided with covers. When metal covers are used, they were not grounded. In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass were not provided with bushings designed for the purpose or they did not have smooth, well-rounded surfaces on which the cords may bear: (a) At the project on or about October 27, 2011, and at times prior thereto, the cover plate was broken on a five gang switch box and there were no covers on three outlet boxes. There was 110 VAC in the boxes.
