29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    (a) At Apra Heights, building 4175, room 103, eye protection was not used while cutting metallic plumbing drain material with a hacksaw. Employee was exposed to the hazards of flying metallic fragments and/or foreign matter to the eyes.

29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    (a) At the southwest side of room 105, trash, air conditioning, and plumbing material cluttered the the path to the entries of the room.  Employees were exposed to tripping over loose debris on the floor.
