29 CFR 1910.95(b)(1):  Employees were subjected to sound levels exceeding those listed in Table G-16 of Subpart G of 29 CFR 1910.95 and feasible administrative or engineering controls were not utilized to reduce sound levels:    Wood Products Manufacturing & Recycling, dba Pro Pallet Inc, at 920 E Collins, Eaton, CO: On 11/3/11, employees were subjected to sound levels exceeding those listed in Table G-16 of Subpart G of 29 CFR 1910.95 and feasible administrative or engineering controls were not utilized to reduce sound levels. Employees recycled and fabricated wooden pallets.  On 11/3/11 one employee operating a pallet dismantler was exposed to noise at a dose in excess of the 100% dose Permissible Exposure Limit (PEL).  The employee was exposed to noise at a dose of 372%.  This was 3.7 times the PEL.  The employer did not implement feasible administrative or engineering controls to reduce noise exposure to below the levels listed in Table G-16.  This condition exposed the employee to a hazardous noise dose.    Wood Products Manufacturing & Recycling, dba Pro Pallet Inc, at 920 E Collins, Eaton, CO: On 11/3/11, employees were subjected to sound levels exceeding those listed in Table G-16 of Subpart G of 29 CFR 1910.95 and feasible administrative or engineering controls were not utilized to reduce sound levels. Employees recycled and fabricated wooden pallets.  On 11/3/11 one employee operating a pneumatic nail gun was exposed to noise at a dose in excess of the 100% dose Permissible Exposure Limit (PEL).  The employee was exposed to noise at a dose of 250%.  This was 2.5 times the PEL.  The employer did not implement feasible administrative or engineering controls to reduce noise exposure to below the levels listed in Table G-16.  This condition exposed the employee to a hazardous noise dose.    Wood Products Manufacturing & Recycling, dba Pro Pallet Inc, at 920 E Collins, Eaton, CO: On 11/3/11, employees were subjected to sound levels exceeding those listed in Table G-16 of Subpart G of 29 CFR 1910.95 and feasible administrative or engineering controls were not utilized to reduce sound levels. Employees recycled and fabricated wooden pallets.  On 11/3/11 one employee operating a trim trac saw was exposed to noise at a dose in excess of the 100% dose Permissible Exposure Limit (PEL).  The employee was exposed to noise at a dose of 183%.  This was 1.8 times the PEL.  The employer did not implement feasible administrative or engineering controls to reduce noise exposure to below the levels listed in Table G-16.  This condition exposed the employee to a hazardous noise dose.

The employee was exposed to noise at a dose of 127%.  This was 2.5 times the Action Level.  The employer did not administer a hearing conservation program.  This condition exposed the employee to a hazardous noise dose.    Wood Products Manufacturing & Recycling, dba Pro Pallet Inc, at 920 E Collins, Eaton, CO: On 11/3/11, Pro Pallet Inc did not administer a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) of this section, where employee noise exposures equaled or exceeded an 8 hour time weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response), or equivalently, a dose of fifty percent. Employees recycled and fabricated wooden pallets.  Employees during the course of their duties were exposed to flying particulates, vehicular traffic, overhead hazards, noise, and lacerations from wooden pallets.  The employer had not performed a written certification of the workplace hazard assessment.

The employer allowed the employees to voluntarily wear Seidman Associates RS-810 comfort masks.  This comfort mask was not a NIOSH certified respirator.  This condition potentially exposed employees to a respiratory hazard.
