The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective.    a) On 11/03/11 at the workplace, in the Cooling Area, a 30" Commercial Duty Circulator, manufactured by Dayton, was ungrounded causing the unit's casing to become electrically live. The grounding pin had been bent and broken loose inside the plug's housing preventing a continuous path.        b) On 11/03/11 at the workplace, in the Packing Area, a 30" Commercial Duty Circulator, manufactured by Dayton, was ungrounded causing the unit's casing to become electrically live. The grounding pin had been broken off or removed from the plug's housing preventing a continuous path.
