29 CFR 1910.1200(h)(1):     Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    As of October 5, 2011, employees operating or in the vicinity of gasoline- or diesel-powered equipment, including but not limitied to concrete buggies and Bobcats, had not been trained on the potential health effects of carbon monoxide.  Employees performing demolition operations of walls or ceilings where the plaster contains silica had not been trained on the potential health effects of crystalline silica.
