29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes.    a) Ohio Mirror Technologies, Inc. - Paulding, Ohio: On or about November 15, 2011, the employer did not assure that employees handling glass for tasks such as, but not limited to, cutting operations wore appropriate personal protective equipment for the hands.  Employees handling, cutting/scoring and breaking glass were exposed to severe lacerations in that gloves were not cut resistant.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

Cardboard stored under the furnace and around the glass bending press presented a fire hazard when near heat sources and hot equipment.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

The employer did not assure that employees successfully completed a forklift operator training program containing elements in 29 CFR 1910.178(l) before an employee(s) was permitted to operate the forklift. The orange power cord was subject to pull from the junction box and exposed employees to electrical hazards.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
