29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it is safe and defective rigging equipment was not removed from service: (a)Kutch Steel Fabrications at 2100 S. Holly St., Denver, CO 80222: The employer did not inspect the rigging material prior to each use and did not remove the defective rigging from service in that the LiftAll sling used to lift steel joists of approximately 800 pounds each was severely frayed. The employer continued to the use the frayed sling which exposed the employees to a struck-by hazard.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or was not designed, installed, and used under the supervision of a qualified person as required by this subpart: (a)Kutch Steel Fabrications at 2100 S. Holly St., Denver, CO 80222: The employer did not ensure the anchorage for the personal fall arrest system was capable of supporting at least 5000 pounds and was designed and installed as required by the standard in that a vertical piece of 5/8 inch rebar was used as an anchor. This condition exposed the employee to a fall hazard of approximately 14 feet.

29 CFR 1926.600(a)(6)(i): The employer did not ensure any part of the crane or load maintained a clearance of 10 feet from lines rated 50 kV or below when working or being moved in the vicinity of power lines or energized transmitters, except where electrical distribution and transmission lines have been deenergized and visibly grounded at point of work or where insulating barriers, not a part of or an attachment to the equipment or machinery, have been erected to prevent physical contact with the lines: (a)Kutch Steel Fabrications at 2100 S. Holly St., Denver, CO 80222: on November 17, 2011 the employer did not ensure the high power transmission lines energized to 7,620 volts and hanging approximately 6.5 feet from the top of the east wall were deenergized, visibly grounded at point of work or were fitted with an insulating barrier. This condition exposed the employees to high voltage electrical hazards while installing steel joists on the walls with the embed plates on the east and west walls.

Holly St., Denver, CO 80222: The employer made modifications in the form of a "jib" type device connected to the mast of the GEHL 1083 forklift to assist in the lifting of steel joists . This modification affected the capacity and operation of the forklift and was made without the manufacturer's written approval. This condition exposed the employees to struck-by hazards and the hazards of an unstable forklift.
