29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a. On or about November 23, 2011, at the above addressed jobsite, employees installing guardrails were exposed to falls, of seventeen (17) feet six (6) inches, created by the presence of unprotected sides.    THE LORIG CONSTRUCTION COMAPNY, WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(b)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314814872, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON OCTOBER 22, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT LAWRENCE AVENUE BRIDGE OVER I-90, CHICAGO, ILLINOIS.

29 CFR 1926.502(d)(6)(ii): Unless the snaphook is a locking type and designed for the following connections, snaphooks shall not be engaged: to each other;    a.  On or about November 23, 2011, at the above addressed jobsite, an employee conducting jack hammering engaged two snap hooks to each other, thereby exposing the employee to the hazards associated with falls.
