29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation.    (a) The employer did not provide the part of the body affected and the object and/or substance that directly injured or made the person ill, for column F of their 2010 and 2011 OSHA 300 Log.    (b) The employer had multiple work sites, which require less than a year of on-site employment. The employer did not provide sufficient information of the location, for column E, where employee injuries occurred in their 2010 and 2011 OSHA 300 Log.
