29 CFR 1926.153(d)(3): Container safety relief devices and regulator relief vents were located closer than 5 feet in any direction from air openings into sealed combustion system appliances or mechanical ventilation air intake. (a) The employer failed to ensure that the fumigator was more than 5 feet from the safety relief valve on the propane tank. This exposed employees to fire hazards.
