29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents. a) At the worksite, the employer did not provide and ensure that employee(s) used eye protection while using a circular saw to cut materials, thereby exposing employee(s) to a potential struck-by hazard.

The alleged violations below have been grouped because they involve similar or related hazards that may increase the potential for injury resulting from an accident or illness. 29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews: a) At the worksite, the employer did not ensure that end rails were installed on the pump jack scaffold that was utilized by employees installing siding at heights greater than ten feet, thereby exposing employees to an appoximate 15 foot fall hazard.

29 CFR 1926.451(g)(4)(iv): When midrails are used, they shall be installed at a height approximately midway between the top edge of the guardrail system and the platform surface. a) At the worksite, the employer did not ensure that mid rails were installed onto the pump jack scaffold that was used by employees to install siding onto a new build at heights greater than 10 feet, thereby exposing employees to an approximate 15 foot fall hazard.
