29 CFR 1926.404(f)(6):   The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    Located on the ground area of Jefferson Street there was a air compressor  connected to the temporayr electrical service with a 50.0 foot orange/black extension cord that was missing a grounding pin exposing workers to potential elecetrocution hazards.

29 CFR 1926.501(b)(13):   Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)    Workers were observed engaged in residential type of construction activities building a multi-story, multi-unit student housing facility. Workers were exposed to potential fall hazards in that proper fall protection systems were not utilized in three areas;  1) Roofers installing roofing material on the upper roof area, 2) Framers were working from the outside window sill area, nailing wooden 4.0x8.0 foot OSB siding material on the 2nd and 3rd story levels.

29 CFR 1926.1053(b)(1):   Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    Located on the Jefferson Street side area, workers were observed engaged in residential type of construction activities building a new multi-story, multi-unit student housing facility. Worker a utilized a 40.0 extension ladder as a means of access and egress. Workers accessing and exiting the roof area were exposed to potential fall hazards, from heights calculated to be 30.0 feet, in that the ladder did not extend 3.0 above the landing surface.
