29 CFR 1910.178(p)(1): Where powered industrial trucks were found to be in need of repair, defective, or in any way unsafe, the trucks were not taken out of service until they had been restored to safe operating condition a) Guam Hardwood Warehouse #2- On or about December 01, 2011 and at times prior thereto, an employee was operating a TMC Industrial Forklift truck model FG30N5, serial number 31M71538 and his vision to see above him was obstructed by the plywood.
