29 CFR 1910.178(p)(1):   Where powered industrial trucks were found to be in need of repair, defective, or in any way unsafe, the trucks were not taken out of service until they had been restored to safe operating condition    a)  Guam Home Center, Warehouse- On or about December 06, 2011 and at times prior thereto, employees operating the Toyota forklift truck, model- 7FDAU50, serial number 70314;  Morfett forklift truck model- M500, serial number 4WD140507; Toyota forklift model- FDU32 serial number 30340; and TMC forklift  model-700030, serial number FHD30G5 had Plexi glass covering the overhead guards obstructing the operator's view when off loading or loading materials.  Also, the TMC forklift truck model- 700030, serial number FHD30G5 had no horn.
