No labels or identifiers such as serial numbers could be located on these pieces of equipment. Machines are located in the production department.       a) Rotaries number 1, 2, 3 and 4. On December 15, 2011 an employee suffered a partial amputation to middle and ring fingers on left hand. This injury occurred while setting up and running initial batch on products on Rotary number 1.    b) Levers: Employees were exposed to the point of operation while using the Levers. The unused portions of the blades on the levers were not guarded.     c) Red Spooler machine: Employees were exposed to the point of operation while using the spooler machine.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
