29 CFR 1910.1096(d)(2)(i):     The employer did not supply appropriate personnel monitoring equipment, such as film badges, pocket chambers, pocket dosimeters, or film rings, and/or require the use of such equipment by each employee who entered a restricted area under such circumstances that he received or was likely to receive a dose in any calendar quarter in excess of 25 percent of the applicable value specified in 29 CFR 1910.1096(b)(1):    a)  The company did not provide technicians holding animals for obtaining x-rays finger ring dosimetry to determine the extremity exposure to the employees when lead glove use would be a greater hazard of injury to the person or animal.

29 CFR 1910.1096(n)(1):     The employer did not maintain records of the radiation exposure of all employees for whom personnel monitoring is required under 29 CFR 1910.1096(d) and did not advise each employee of individual exposure on at least annual basis:    a)  The company performed whole body quarterly monitoring of radiation exposure during x-ray operations with animals being treated but never informed the individual employees of their exposure on an annual basis.

29 CFR 1910.1200(e)(1)(i):     The written hazard communication program did not include a list of the hazardous chemicals known to be present, using an identity that was referenced on the appropriate material safety data sheet:      a)  The company hazard communication program did not include a list of the hazardous chemicals being used by employees.  The computer internet access being used to retrieve sheets for employees if requested on chemicals such as antineoplastics, medications, isopropanol, sanitizers, disinfectants and cleaners.
