29 CFR 1904.4(a):     Basic requirement. Each employer required by this Part to keep records of fatalities, injuries, and illnesses must record each fatality, injury and illness that is (a) work-related; (b) a new case; and (c) that meets one or more of the general recording criteria of 29CFR 1904.7 or the application to specific cases of 29CFR 1904.8 through 1904.12.     The employer did not establish an OSHA 300 Log for 2011 when injuries resulting from lost work days were documented such as but not limited to:    a) An employee received a cut on the arm with a box knife on or about August 23, 2011 and was off work until on or about September 16, 2011;    b) An employee received a cut on the arm with a chop saw on or about August 21, 2011 and was off work until on or about October 31, 2011.

29 CFR 1910.1200(g)(8):     The employer shall maintain in the workplace copies of the required material safety data sheets foreach hazardous chemical, and shall ensure that they are readily accessible during each work shift to employees when they are in their work area(s).(Electronic access, microfiche, and other alternatives to maintaining paper copies of the material safety data sheets are permitted as long as n obarriers to immediate employee access in each workplace are created by such options.)    The employer did not maintain copies of material safety data sheets (MSDS) in that the employer did not have a MSDS for propane.
