29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: The employer did not develop, document, or utilize machine specific procedures for the control of potentially hazardous energy when conducting equipment servicing and/or maintenance. Equipment includes, but is not limited to, screw machines, CNC lathes, CNC milling machines, etc. NOTE: Procedures shall clearly and specifically outline the scope, purpose, autorization, rules and techniques to be utilized for the control of hazardous energy and the means to enforce compliance including the following: 1) A specific statement of the intended use of the procedure; 2) Specific procedural steps for shutting down, isolating, blocking, and securing specific machines or equipment to control hazardous energy; 3) Specific procedural steps for the placement, removal, and transfer of lockout devices or tagout devices and the responsibility for them, and 4) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other control measures.

(b) The 8 Brown & Sharp single screw machines did not have the side guarded. An employee was observed reaching through the side of the machine to adjust a cooling hose.
