29 CFR 1904.39(a):  The employer did not report a work-related fatality to OSHA within eight (8) hours after the death of an employee from a work-related incident:    (a)O'Brien Concrete Pumping - Colorado Inc., at the 1000 Block of Galapago St., Denver, CO 80223:  The employer did not report the work-related death of an employee within 8 hours.  The accident occurred at approximately 9:36PM on February 8, 2012 and the employer did not report the incident to OSHA.  The Area Office became aware of the fatality by a media article on February 13, 2012.
