Two employees were pumping concrete onto the second floor metal deck in the west side of Area E. The material placement system included a 104' long swiveling boom with a 5" metal pipe and a 4" reducer, and up to 60 feet of 4" horizontal hose, pumping concrete under pressure. The boom and hose contained stored energy while pumping under pressure, or when moving the boom and hose. Employees were not trained in the safe handling of this material delivery system.
