29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.    a)  O'Brien Concrete Pumping - Colorado, Inc. at 1200 Hidden Valley Parkway, Severance, CO 80550:  On April 4, 2018, the employer did not ensure that employees operating a Putzmeister concrete pump truck and material delivery system were qualified by training or experience to prevent striking employees or structures.  Two employees were pumping concrete onto the second floor metal deck in the west side of Area E.  The material placement system included a 104' long swiveling boom with a 5" metal pipe and a 4" reducer, and up to 60 feet of 4" horizontal hose, pumping concrete under pressure.  The boom and hose contained stored energy while pumping under pressure, or when moving the boom and hose.  Employees were not trained in the safe handling of this material delivery system.  This condition exposed employees and other employees working in and around these areas to struck-by hazards.
