Each employee on ramps, runways, and other walkways shall beprotected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems. a. On the site, the area around the sludge basin was not protected by a guardrail system thereby exposing employees to a 12 foot fall hazard.

29 CFR 1926.651(j)(2): Employees shall be protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations. Protection shall be provided by placing and keeping such materials or equipment at least 2 feet (.61 m) from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. a. On the site beside the Aeriation B Tank where employees were placing guardrails to be installed on the tank, the spoil pile was placed directly beside the trench thereby exposing employees to a struck-by hazard.
