A separate, confidential list of the case numbers and employee names for privacy concern cases was not kept.    Oneworld Community Health Center, Inc--Omaha:  The employer recorded employee names where injuries/illnesses involved privacy concerns; a separate confidential log was not kept as required by this regulation and 29 CFR 1910.1030(h)(1).  Instances include, but are not limited to, recordable needlestick injuries on the 2009, 2010, and 2011 OSHA 300 logs.
