29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) A Hazardous Energy Control Program was not developed to ensure that employee(s) who perform lockout tagout or are affected by lockout tagout procedures are not exposed to unexpected energizing, startup or release of stored energy from mechanical and electrical components associated with, but not limited to lathes, saws, air compressor, local exhaust and dust collection system.

29 CFR 1910.212(a)(1):     Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts::    (a) Employees operate a Fryer Easy Turn CNC lathe with no guarding in place to prevent the operator from coming into contact with the rotating chuck and indexing tool turret.  This exposed the operator to a caught-in/on hazard.    (b) Employees operate a LeBlond Lathe without a chuck guard to prevent the operator from coming into contact with the rotating chuck and part. This exposed the operator to a caught-in/on hazard.    (c) Employees operate a Romi - Bridgeport EZ Path CNC lathe without guarding in place to prevent the operator from coming into contact with the rotating chuck. This exposed the operator to a caught-in/on hazard.    (d) Employees operate a Bridgeport -Torqu-Cut 22 CNC machine that did not have the rotating tool turret guarded. The sliding door that enclosed the milling process was not interlocked, exposing the operator to a caught-in/on hazard.    (e) Horizontal band saw (Ksaw) did not have the unused portion of the blade guarded, exposing operators to a laceration and/or amputation hazard.

(b) Employees use the Air Knife unit 2 to cut braided rope material, without point of operation guarding in place, exposing the operators to a laceration and amputation hazard from the cutting blade.    (c) Employees use the Rockwell table saw to cut phenolic resin sheets without point of operation blade guarding in place, exposing the operators to a laceration and amputation hazard.
