29 CFR 1918.62(b)(3)(i):     Ten randomly distributed broken wires in one rope lay or three or more broken wires in one strand in one rope lay;    Bow storage area:  On or about February 11, 2012 and times prior thereto, a four way wire rope sling used to load/offload supplies had one entire lay broken with multiple other strands broken.  This exposed employees to struck-by hazards from one of the legs breaking and possibly dropping the load on employees.

29 CFR 1918.62(a)(2):     Defective gear, as defined by the manufacturers' specifications (when available), shall not be used. Distorted hooks, shackles or similar gear shall be discarded.    Pavlof weather deck and dock side:  On or about February 12, 2012 and times prior thereto, brailer bags were used with cuts and snags on the bag and lifting legs.  This exposed employees to struck-by hazards from defective gear failing during offloads of up to 2000 ilbs of fish.
