29 CFR 1926.404(b)(1)(ii):     On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection.      a)  Construction site office, south end of building - The two 120 vac 15 amp receptacles did not have GFCI protection, on or about 01/30/12;    b)  Ground floor hallway, south end of building - Two of the 4 receptacles in the 4x4 utility box did not have GFCI protection, on or about 01/30/12.    NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a)  2nd Floor level, East edge of building, above entrance - Employee without fall protection at the edge of the building, approximately 24' above the ground level, on or about 01/24/12;    b)  2nd Floor level, South edge of building - Employee without fall protection at the edge of the building, approximately 24' above the ground level, on or about 01/24/12;    c)  2nd Floor level, East edge of building - Employee without fall protection at the edge of the building, approximately 24' above the ground level, on or about 01/26/12.         NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
