29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to their work environment to control or eliminate any hazards or other exposure to illness or injury:    a)  Jobsite, 113 Route 73, Voorhees, NJ - Employees making repairs to a light in the facility parking lot did not recognized the unsafe conditions of improperly operating the JLG Model G6-42A Telehandler to lift an employee without fall protection approximately 10-12' above ground; they did not recognize the lift operator walking away from the lift with somebody on an elevated platform as unsafe; they did not recognized the way they were using an unsecured wooden pallet as a scaffold platform as unsafe; they did not recognized not having fall protection as unsafe, they did not recognize the need for head protection, on or about 02/04/13;    b)  Jobsite, 113 Route 73, Voorhees, NJ - Employees making repairs to a light in the facility parking lot were not familiar with the regulations that apply to safely operating the JLG Model G6-42A Telehandler when lifting an employee without fall protection approximately 10-12' above ground; were not familiar with the regulations requiring the lift operator to stay at the controls of the lift while somebody on the elevated platform; were not familiar with the regulations for the proper use of scaffolding; were not familiar with the regulations requiring fall protection when working at heights greater than 10 feet; were not familiar with the regulations requiring head protection, on or about 02/04/13.            NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
