29 CFR 1910.119(j)(2):     Written procedures. The employer shall establish and implement written procedures to maintain the on-going integrity of process equipment.      The employer did not implement a management of change procedure when it was decided to operate the Chlorate Destruct tank without a functioning rupture disk.
