29 CFR 1915.89(c)(7)(iv): The employer did not ensure the lockout/tags-plus coordinator maintained and administered a continuous log of each lockout and tags-plus system: Factory Deck: On or about February 16, 2012 and at times prior thereto, the Lockout/Tags-plus log was not annotated for the lockout that had been done for worker on the Baader machines. This condition exposed employees to inadvertent start-up of the equipment resulting in caught-in and amputation injuries.

29 CFR 1918.103(a): The employer did not ensure that each affected employee wore a protective helmet when working in areas where there was a potential for injury to the head from falling objects: Freezer Space, Offload: On or about February 16, 2012 and at times prior thereto, employees working under conveyors in operation were not wearing protective helmets. This condition exposed employees to struck by hazards from the 44 pound boxes of fish product that fell off the conveyors.

29 CFR 1918.101(a)(1): The employer did not ensure that each affected employee used appropriate eye and/or face protection where exposures to eye and/or face hazards: Freezer Space, Offload: On or about February 16, 2012 and at times prior thereto, employees did not utilize appropriate eye protection. This condition exposed employees to eye hazards from using pry bars to remove wooden pallets used as cargo spacers.
