Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards:     a)	Kubat Equipment & Service at Bradley Gas Station 2101 S. Holly St., Denver, CO 80222: On 2/27/12, the employer did not ensure that employee was wearing a seat belt while operating a Yale model Veracitor 50VX forklift. This condition exposed the operator to serious injuries in the event of a tip-over or other accident.     Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
