29 CFR 1910.23(a)(3)(ii):     Every hatchway and chute floor opening shall be guarded by:    A removable railing with toeboard on not more than two sides of the opening and fixed standard railings with toeboards on all other exposed sides. The removable railings shall be kept in place when the opening is not in use.  Where operating conditions necessitate the feeding of material into any hatchway or chute opening, protection shall be provided to prevent a person from falling through the opening.    In the Pulper Area at the workplace, on 02/29/12, employees were exposed to a chute opening that did not have it's removable railings kept in place while the opening was not in use.

29 CFR 1910.147(d)(4)(i):     Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees.    In the conveyer area on February 27, 2012, at the workplace, lockout or tagout devices were not affixed to all energy isolating devices as required by Ohio Paperboard's written Lock Out Tag Out Procedure while performing maintenance on the conveyer.
