29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a/ Petro Packaging Company, Inc./Cranford NJ Site - Building # 16: Employees are required to repair and service the equipment used in the process. The employer did not establish an energy control program to assist/protect employees performing any work/service on the equipment. The program to include written procedures, machinery, type of energy, lock out devices to be used, training and annual inspections of the program. Equipment to include but not limited to:    1/ Injection Molds 1, 2 and 3   2/ Extrusion lines 1 to 5  3/ Air compressor   4/ Cap machine       Condition observed on/or about 2/24/12.

29 CFR 1910.178(l)(4)(iii):     An evaluation of each powered industrial truck operators performance was not conducted at least once every 3 years    a/ Petro Packaging Company, Inc./Cranford NJ Site - Building #16: Powered industrial truck operators were not evaluated at least once every three years. Evaluation to include, but not limited to driving skills, review of safe operations, etc. The evaluation shall be documented to include the name of the operator evaluated, the name of the person conducting the evaluation and the date of the evaluation.     Condition observed on/or about 2/24/12.
