29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift. (a) Phoenix Sign Installations, LLC. on or about March 01, 2012, at the address of 2314 Harding Hwy. Lima, OH: The employer failed to ensure an employee working from an articulating truck mounted manlift was adequately protected from falls. The employee replacing a sign from the lift in this condition was exposed to an approximately 10 ft fall to the pavement below.
