29 CFR 1926.1412(f)(2): The equipment was not inspected by a qualified person at least every 12 months in accordance with paragraphs (f)(2)(i) -- (xxi) of this section.     At the site, on March 2, 2012, where the Hydralift model HL-350 S/N 35303982AAR1 crane was being used to remove a sign, the crane did not have an inspection within the last 12 months.

29 CFR 1926.1433(d)(4)(i): Hooks were not equipped with latches, except where the requirements of paragraph (d)(4)(ii) of this section are met.    At the worksite, on March 2, 2012, where the Hydralift model HL-350 S/N 35303982 AAR1, was used to remove a sign, the safety latch was removed from the hook.  This condition exposed employees to struck-by hazards.
