OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck by hazards from falling loads while transporting rolls of paper. a.) Erdie Industries, Inc. located at 1205 Colorado Ave. Lorain, OH; On or about March 8, 2012, an employee was exposed to a struck-by, crushing hazard from a falling load due to the missing latch over the throat of the hook for the Meco Matic 10-ton overhead top running crane in the Shop Area when carrying paper rolls ranging in weight from 3,800 to 6,000 pounds onto the slitter. Among other methods, a feasible and acceptable abatement method to correct this hazard is to follow the American Society of Mechanical Engineers (ASME) B30.2-2005 Overhead and Gantry Cranes (Top Running Bridge, Single or Multiple Girder, Top Running Trolley Hoist), Section 2-1.14.5 Hooks.

Erdie Industries, Inc. located at 1205 Colorado Ave. Lorain, OH; On or about March 8, 2012, the employer did not assure an energy control program consisting of employee training, periodic inspections and providing lock out devices, was established to prevent caught by and amputation hazards while adjusting the blades on the Cameron Paper Slitter. b.) Erdie Industries, Inc. located at 1205 Colorado Ave. Lorain, OH; On or about March 8, 2012, the employer did not assure an energy control program consisting of employee training, periodic inspections and providing lock out devices, was established to prevent caught by and amputation hazards while changing the bullets and adjusting the blades on the Pack Industrie Tube Winder.
