29 CFR 1910.253(b)(4)(iii):  Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:    a)  On or about 03/15/2012, Priority Ro-Ro, Isla Grande Oeste-Panamerican Dock, Mechanic  shop area, San Juan PR:  Employees worked and walked by acetylene & oxygen gas cylinders which were stored together, left standing near a wall and were not separated .    NOTE:   Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.

29 CFR 1917.94(a): The employer did not ensure that employees exposed to falling or rolling objects, or objects piercing the sole wore protective footwear.    On or about 03/15/2012, Priority Ro-Ro, Isla Grande Oeste-Panamerican Dock, Mechanic  shop area, San Juan PR.:  Employees were installing intermodal container tires to a metal truck rims using heavy tools without wearing safety shoes so as to protect them from injury.     Note:  Certification and Documentation of abatement must be submitted for this citations item as required by 29 CFR 1903.19(c) and (d), respectively

29 CFR 1917.111(a): The structural integrity of docks, wharves, piers, terminals and/or working surfaces was not maintained.    On or about 03/15/2012, Priority Ro-Ro, Isla Grande Oeste-Panamerican Dock, Yard area, San Juan PR.  Employees engaged in the stevedoring operations were exposed to struck by, caught by and fall hazards as evidenced by the by the terrain conditions on several areas at the marine terminal. Several holes and unleveled terrain were observed where hustler trucks (brutes), with or without trailers, were moving while performing stevedoring operations.    Note:  Certification and Documentation of abatement must be submitted for this citations item as required by 29 CFR 1903.19(c) and (d), respectively
