29 CFR 1926.451(c)(1):  Supported scaffold(s) with a height to base width (including outrigger supports) ratio greater than four to one (4:1) was not restrained from tipping by guying, tying, bracing, or equivalent means:    a)  Employees were exposed to fall hazards of greater than 25 feet while working on a frame scaffold which was not restrained from topping by any means.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(g)(1):  Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level:    a)  Employees working from frame scaffolding were exposed to fall hazards of greater than 25 feet.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
