29 CFR 1910.212(a)(2):     Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:    At the worksite, on March 15, 2012, where the access panel for the CNC Lathe 201 (model number 200LC, S/N PH200738, was removed, exposing the rotating shaft and chuck.  This condition exposed employees to caught-by hazards.     At the worksite, on March 15, 2012, where the access panel for the CNC Lathe 202 (model number DF 80000, S/N 0012), was removed, exposing the rotating shaft and chuck.  This condition exposed employees to caught-by hazards.     At the worksite, on March 15, 2012, where the access panel for the CNC Lathe 221 (unknown model number, unknown S/N), was removed, exposing the rotating shaft and chuck.  This condition exposed employees to caught-by hazards.

29 CFR 1910.215(a)(4):     Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:    Inside the machine shop, on March 15, 2011, where the Baldor grinder (model  G630117, serial number  W0610310910) was used to grind parts:    The left work-rest was adjusted approximately 7/8 inches away from the grinding wheel.      The right work-rest was adjusted approximately 1 and ? inches away from the grinding wheel.
