29 CFR 1910.147(c)(5)(ii): Lockout devices and tagout devices shall be singularly identified; shall be the only device(s) used for controlling energy; shall not be used for other purposes:    (a)	Lowes HIW, Inc. DBA Lowes at 215 Meadow Lane, Glenwood Springs, CO 81601: On and before 3/7/12, the employer did not affix a singularly identified lockout device for the control of hazardous energy when employees were engaged in activities covered by this section. Employees maintained the trash compactor in the receiving area. Servicing may be performed by outside contractors. The equipment is powered by hazardous energy sources. Lowes had not developed and documented a deenergization procedure for the equipment. This condition potentially exposed employees and employees of contractors to hazardous energy sources. DBA Lowes at 215 Meadow Lane, Glenwood Springs, CO 81601: On and before 3/7/12, the employer did not ensure that lockout or tagout devices were affixed to each energy isolating device by authorized employees. Employees did not affix lockout or tagout devices to the breaker panel in the electrical room. Servicing may be performed by outside contractors. The equipment is powered by hazardous energy sources.  This condition potentially exposed employees and employees of contractors to hazardous energy sources. DBA Lowes at 215 Meadow Lane, Glenwood Springs, CO 81601: On and before 3/7/12, the employer did not ensure that the 480v electrical panel in the electrical room had all breakers clearly marked to correspond with their actual equipment purpose or function.
