29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    a)  On or about May 9, 2013, and at times prior, employees who changed the panel saw blade in the lumber department were exposed to amputations because the employer did not conduct a periodic inspection of the energy control procedure.  Employees had been changing the saw blade without following company lockout procedures and without applying an energy isolating device.     Lowes HIW, Inc. dba Lowes Home Center, Inc., was previously cited for a violation of Occupational Safety and Health Standard 29 CFR 1910.147(d)(6)(i) or its equivalent which was contained in OSHA inspection 191400, Citation 1, Item 1, issued April 16, 2012, with respect to a workplace located at 15601 S. Lagrange Road, Orland Park, IL 60462.  This citation became final on May 6, 2012.  The final abatement date for this citation was May 7, 2012.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:  (a)  On or about May 9, 2013, and at times prior, the employer did not establish a written respiratory protection program and provide training to employees who voluntarily used respirators while handling hazardous waste.  Employees used elastomeric half-face respirators with organic vapor cartridges without being provided medical evaluations and basic training in respirator use, cleaning, storage and maintenance which ensured that the use of the respirators themselves did not present a health hazard to the employees.
