Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushed by and struck by hazards relating to the use of automotive lifts:    a)  Automotive lifts were not maintained by qualified and train individuals.  The Nussbaum lift was not repaired and maintained by a qualified individual. Two Gray 2-Post lifts were not maintained properly resulting in the locking mechanism for the lift arms not functioning properly, allowing the arm to kick out and the vehicle to shift and/or fall creating a struck by and/or crushed by hazard.      b)  The employer did not provide operator lift training to employees.      c)  The employer did not provide an inspection program including inspection points.      d)  The employer modified equipment without the express written permission of the manufacturer.  The Rotary Lift in the alignment bay had a screwdriver used to hold the locking mechanism in the place of the pin provided by the manufacturer.  This provided a crushed by and/or struck by hazard for employees working near the lift.      ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
