29 CFR 1910.101(b):     The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which was incorporated by reference as specified in 29 CFR 1910.6:    The employer does not require the storage of gas cylinders in accordance with Compressed Gas Association Pamphlet P-1-1965. This violation was observed on or about March 23, 2012 in the warehouse area where employees were exposed to a struck by hazard when required to enter/exit the bay door area with unsecured compressed gas cylinders.

29 CFR 1910.132(a):     Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered    The employer does not require employees to don seatbelts when operating forklifts.  This violation was observed on or about March 23, 2012 in the warehouse area where employees were exposed to an ejection hazard while operating a Toyota C20 forklift without donning a seatbelt.

29 CFR 1910.178(l)(2)(ii):     The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    The employer does not train operators in the safe operation of powered industrial trucks.  This violation was observed on or about March 23, 2012 in the warehouse area where employees were exposed to a struck by hazard when required to operate the Toyota C20 forklift without formal instruction and practical training.
