29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    a) On or about March 29, 2012 and prior to that time located along various areas of the residential structure employees performed or were observed performing framing activities adjacent to an unprotected stairway landing and performed joist band board framing an exterior wall layout adjacent to unprotected perimeter exterior floor areas without conventional fall protection or alternative fall protection systems and were exposed to fall potentials in excess of 16'.

29 CFR 1926.1052(c)(1):  Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:    a) Located along the central area of the residential structure employees were observed utilizing a 16 step high set of stairs which was not protected by standard stair railing on one unprotected side or edge exposing employees to fall potentials in excess of 8'.
