29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by: point of operations and rotating parts and/or flying chips or sparks. The employer does not ensure that CNC machines with interlocks are working in the safe operation. This violation occured and was observed at worksite located at 11633 Spring Cypress, Tomball, Texas on or about March 19, 2012, and times prior thereto, where employees, who were operating the following Vertical Lathes without working interlocks and CNC Lathes without chuck guards: Vertical Lathes 1) Yang SMV1000 #106, 2) Okuma #105 3) Okuma LC40 4) Okuma #104 CNC Lathes w/o Chuck guards 5) Kingston HD30 #111 6) Summit Lathe #112

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: In manufacuturing faciility; the employer did not develop or implement an energy control program where employees are exposed to electrical and pneumatic energy devices. This violatoin was observed on or about 3/19/2012 in the manufacturing facility where employees are conducting servicing and maintenance activities on the following 3-phase voltage equipment with, but not limited to, electrical and pneumatic energy devices. this condition exposed employee to struck-by hazards and caught-in hazards. 1) Cincinnati Milacron, 480 Volts, Pnuematic 2) Honeoye, 220 Volts 3) Webb 2VH, 220 Volts 4) Sumith Lathe, #112, 220 Volts, 5) Kingstone HD 30 Lathe, #111, 440 Volts 6) Mazak Powercenter, 208 Volts, Pneumatic 7) Yang SMV1000, #106, 208 Volts, 45 - 4000 RPM, Pneumatic 8) Bullard, 480 Votls 9) Ikeda Drill, 480 volts 10) Okuma LC40, #105, 208 Volts 11) Okuma LH55, #104. 208 Volts 13) Okuma #103, 208 Volts 12) Puma 12, 208 Volts 13) Puma 700, 208 volts 14) Mazak 400, 460 volts, Pneumatic 15) Mazak 1060V, #100, 460 volts, Pneumatic
