29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system.     WORKSITE: The employees using a ladder jack scaffold to access the roof were not protected from falling approximately 20 feet by the use of guardrails or other safe means.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     WORKSITE: The employees, engaged in roofing activities on the 5 in 12 pitch roof were not protected from the hazard of falling approximately 21 feet to the ground by the use of appropriate personal fall arrest systems and/or other effective safety measures.    Tim McCarthy Roofing LLC was proviously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1926.501 (b) (10), which was contained in OSHA inspection number 312971 856, citation number 1, item number 1a, issued on 4/21/2009 and affirmed as a final order on 5/6/2009 with respect to a workplace located at 171 Main Street, Cheshire, CT 06410.
