29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  JOBSITE:  The employees, using an Werner 28 ft. aluminum extension ladder to have access to an upper working level were not provided with information and training regarding the safe use and handling of the ladder.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support:    JOBSITE:  The horizontal distance to the vertical support to the base of the 28 ft. aluminum extension ladder which was used by the employees to access the roof of a house for roofing work, was more than one fourth (1/4) the working length of the ladder.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    JOBSITE:  The employees, engaged in roofing activities on the 3 in 12 pitch roof were not protected from the hazard of falling between 8 feet 4 inches to 10 feet 2 inches to the ground by the use of appropriate personal fall arrest system and/or other effective safety measures.     Tim McCarthy Roofing LLC was previously cited for a violation of this Occupational Safety and Health standard {1926.501(b)(13), which was contained in OSHA Inspection Number 329077, Citation Number 02, Item Number 001 and was affirmed as a final order on 08/13/2012, with respect to a workplace located at 520 Main Street, Cannonfield #19, Ridgefield, CT 06877.
