On or about April 3, 2012, at the above addressed jobsite, an inspection was not conducted by a competent person capable of correcting hazards, such as falls and missing excavator windows, which could result in employee injuries.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On or about April 3, 2012, at the above addressed jobsite, an employee performing demolition operations from the second floor was exposed to a fall, of ten (10) feet nine (9") inches, created by the presence of unprotected sides.

On or about April 3, 2012, at the above addressed jobsite, an employee was conducting demolition operations in a Caterpillar 330C hydraulic excavator without cab glass, thereby exposing the employee to the hazards associated with being struck-by materials.
